Please fill out all 6 forms
Triplicate, yellow, green, white, pink, and blue. If you have more then one child you need the first 4 for each child and you can use the pink and blue forms for the whole family.

Cost

On January 24, 2010: Save $10 per child

Thru February 20: $110 for the first athlete of family, $110 for the second & $80 for the third.

February 21 to March 13: $135 for the first, $135 for the second & $100 each additional.
After March 13: (if allowed by the league): $150 each athlete.

Practice Clothes
Your practice shirt with your FIRST NAME printed on it, is REQUIRED at all practices. Also, and we can’t say it enough times, ALWAYS WEAR SWEATS TO PRACTICE. Either the ones we gave you or your own.

Extra Clothes
T-shirts are $7. Practice sweat pants are $10. We have adult sizes in most items if you want to match your kids. Valley Raiders logo warm-up suit: tba

Checks
Can be made payable to VRTC. Returned checks will incur a $25 fee.

Refunds
Notification:
Thru March 1, 2010: Full refund less clothing received $10/shirt or sweats.
March 2 to March 13: 50% refund
After March 13: No refund.
Multiple Kids: Refund lower kids’ amounts first, as listed above.

NO MATTER HOW OLD THEY ARE!!
They are agreeing to 1 through 7 only, not 8. Your uniform is new and you get to keep it.
